At Pieces of the Puzzle, LLC (“Pieces of the Puzzle”), we believe that privacy is fundamental to everyone, but in particular, those with disabilities. We pledge to always treat your information with the utmost respect and sensitivity and that we will never share your information without your explicit consent, except as outlined in this Privacy Policy below. By using our web site (“Site”) (or requesting products or services from us), you agree to the terms of this Privacy Policy and agree to accept it and all other notices from us electronically at this Site. If you have objections to the Privacy Policy, you should not access or use the Site, or request products or services from us.


HOW WE MAY USE AND SHARE YOUR INFORMATION

We may use or share your information for:
  • Treatment (such as sending sharing information with your therapist(s))
  • Payment (such as billing your insurance)
  • Operational Improvement (such as teaching new hires or measuring our performance).
  • Business Associates (such as a collection agency)

  • Treatment Alternatives
  • Other People Involved In Your Care (such as a family member or friend who helps with your therapy)
In limited cases, we may use or share your information without your permission. We may give out information about you for:
  • Reports of abuse, neglect, or domestic violence;
  • Emergency situations that require immediate release of information; and
  • Police requests and/or court orders.

ASSIGNMENT

If Pieces of the Puzzle goes through a business transition, such as a merger, acquisition by another company, or sale of all or a portion of its assets, we reserve the right to transfer (assign ownership of) your personal information, which will likely be among the assets transferred. If, as a result of the transfer, your information will be used in a manner substantially different from that stated at the time of collection, you will be given notice.

  • Right To Inspect and To Receive Copies. You have the right to look at and to receive copies of the information used to make decisions about your case. To look at and to receive copies of information used to make decisions about you, you must submit your request in writing.

To ask for a change to your record, you must make your request in writing and submit it to the correspondence address listed on this page. Also, you must give a reason that supports your request. We may say no to your request for an amendment to your record. We may do this if it is not in writing or does not include a reason to support the request. We also may say no to your request if you ask us to amend information that:

    • we did not create, unless the person or entity that created the information is no longer available to make the amendment;
    • is not part of the records used to make decisions about you;
    • is not part of the information which you are permitted to inspect and to receive a copy; or
    • is accurate and complete.


  • Right To an Accounting of Disclosures. You have the right to get a list of the disclosures we made of your information. This list will not include all disclosures that we made. You also have the right to ask for a limit on the information we share with someone who is involved in your therapy or in the payment for your therapy. Such a person may be a family member or friend. We do not have to agree to your request. If we do agree, we will fulfill your request unless the information is needed to provide you with emergency treatment. To ask for restrictions, you must make your request in writing us at the correspondence address listed on this page. You must tell us:

    • what information you want to limit,
    • how you want us to limit the information, and
    • to whom you want the limits to apply.


  • Right To Request Confidential Communications. You have the right to ask us to communicate with you about matters in a certain way or at certain places. You must make your request in writing at the correspondence address listed on this page. We will fulfill all reasonable requests.


  • Right To a Paper Copy of This Notice. You may ask us to give you a copy of this Notice at any time. Even if you have agreed to get this Notice electronically, you still have a right to a paper copy of this Notice.


  • Revisions To This Notice. We may update this Notice to show any changes in our privacy practices. We reserve the right to make the updated Notice effective for information we already have about you. It also will be effective for any information we receive in the future. PRIVACY PRACTICES SPECIFIC TO OUR WEBSITE


There are certain areas of the Site that request specific personal information including name, phone number, and e-mail address. Our intent is to allow you as much control as possible over your personal information. By using our Site or requesting products or services from us, you agree to the terms of this Privacy Policy and agree to accept it and all other notices from us electronically at this Site. If you have objections to the Privacy Policy, you should not access or use the Site.


WHAT INFORMATION WE COLLECT ON OUR SITE

As a visitor to this Site, you can engage in activities without providing any personal information. In connection with some Services, Pieces of the Puzzle may ask you to provide certain information about yourself to register a username and a password. TRACKING DATA

Our Site uses the Zorble service and cookies to measure traffic.


THE ZORBLE SERVICE

Our Web pages contain electronic images (called a "pixel image" or "web beacon") that allow us to count page accesses. These pixel images do not collect, gather, monitor or share any personal information about our users (or those of our customers) - they are just the technique we use to compile anonymous, aggregated statistics about site usage. These compiled aggregate statistics are used to improve site performance and experience for users. This technology may be used for tracking purposes such as tracking visitor click stream behavior throughout a website.


USE OF YOUR INFORMATION COLLECTED ON OUR SITE

OUR COMMITMENT TO CHILDREN'S PRIVACY

Protecting the privacy of the very young is especially important. For that reason, we never collect or maintain information from the Site from those we actually know are under 13, and no part of the Site is structured to attract anyone under 13.
